LIVINGSTON ~ LIVINGSTON MANOR ~

RICH IN HERITAGE

 Livingston County has a very early history of being dedicated to the well being of its residents. In 1859 the founding fathers decided the County needed to establish a home to care for the needy. They purchased 240 acres south of Pontiac for this purpose.  The railroad and eventually Route 66 went through the property. This property was to be farmed in order to support the home.

 The first home was built in 1860. Several buildings were added, as the need became apparent. In 1896 a newer T shaped three-story building was constructed to keep up with the need for additional space and the changing times. Many people will remember, when driving down Route 66, years ago Livingston Manor was a very impressive brick building with the big porch and large concrete steps leading to the second floor.

 In the 1950’s the County Supervisors began planning a whole new building to meet the expanding needs. The present building began with the south Wing, attached to the 1896 building by an above ground tunnel. Several buildings, including the stately cattle barn had to be torn down to make way for the completion of the additions, which were completed by 1965. Livingston County was well advanced over most counties in providing such a find facility. The stately old building was then demolished in 1970.

 In 1990, serious consideration was beginning for an addition of a second dining room and much needed office and service areas. This was completed in the summer of 1993. Livingston Manor then looked at the medical needs of its residents and how they were changing. This encouraged the home to seek a skilled care license and now received new certifications in Exceptional Care through Public Aid and also Medicare. Livingston manor has continued to serve the residents of the county with professional and kind understanding for family needs at a very difficult time in their life.

 Our heritage has deep roots in this area. Our pride is in dedicated care.

ADMISSION POLICY

It is the policy of Livingston manor to admit and to treat all residents without regard to race, color, ethnic group, age, gender or disability. The same requirements for admission are applied to all, and residents are assigned within the nursing home without regard to race, color, ethnic group, age, gender or disability. There is no distinction in eligibility for or in the manner of providing any resident service provided by or through the nursing home. All facilities of the nursing home are available without distinction to all residents and visitors, regardless of race, color, ethnic group, age, gender or disability. All persons and organizations that have occasion either to refer an individual for admission or recommend Livingston Manor, are advised to do so without regard to the persons race, color, ethnic group, age, gender or disability. In compliance with Section 504 of the Rehabilitation Act, any resident, representative, prospective resident, or staff member who has reason to believe that he or she has been mistreated, denied services, or discriminated against in any aspect of services or employment because of a disability may file a grievance with the Section 504 coordinator. ADMISSIONS

 Where do I go now?  What should I do next?  Who can answer all my questions and help me make an informed decision about nursing home care? Livingston Manor’s Admission Coordinator will assist you in making whatever arrangements are necessary to assist in the admission process. We recommend you make an appointment or just stop out, take a tour, and talk about he necessary steps you need to take to prepare to enter yourself or a loved one into a nursing facility.

 There are many forms that need to be completed prior to admission to any nursing facility. You will be asked to complete an application as well as to complete and sign a contract for care. These forms and contracts are required for all nursing facilities. If needed we are available to provide consultation in order to assist in the completion of such forms. We accept various payment sources and want to make sure you have a thorough understanding of any financial arrangements and commitments. Financial arrangements need to be made with the bookkeeping office either prior to or at the time of admission. This ensures that we have correct information for billing and other financial issues such as trust accounts. A universal pre-screening process must be completed prior to admission by the local health department. Once again we urge you to plan ahead and let our admissions staff guide you through this process. Room rates vary among facilities. Our Admissions Coordinator works closely with area hospitals and home health services to ensure a smooth an organized admission.

 NURSING SERVICES

 Livingston Manor is a 122-bed facility license by the Illinois Department of Public Health as a Skilled and Intermediate nursing care facility. Individualized medical care is under the direct supervision of a Medical Director and Nursing Services Department comprised of Registered Nursing, Licensed Practical Nurses, Certified Nursing Assistants, and Nursing housekeepers. Resident physicians are available 24 hours a day and make route visits as required by Federal Regulations. Pharmacy Services are also provided and available 24 hours a day. Our Rehabilitation services consist of a Certified Rehabilitation Nurse and several certified rehabilitation aides in the areas of Physical and Occupational Therapy. We are contracted with a local hospital for Registered Physical, Occupational, Speech, and Respiratory Therapy.  All of these services are designed to meet the individual needs of each resident. Hospice care is also available for that resident who qualified Our Medicare Certification allows us to specialize in those individuals requiring short-term rehabilitation in a structured nursing setting.

  DIETARY

 Our dietary department and registered dietitian thoroughly assess each resident’s dietary needs through medical records, lab reports and personal interviews with residents and families. Special diet needs, likes and dislikes, weight losses or gains, loss of appetite, and problems with chewing and swallowing are all monitored on a daily basis. When difficulties with chewing and swallowing are noted, a speech therapist is available on a contracted basis to evaluate a resident’’ needs. Our dietary and nursing staff work closely together to monitor any changes in a resident’s needs.  A Registered Dietitian works with our Certified Dietary manager to plan seasonal menus that provide the recommended daily amounts of protein, carbohydrates, fruits and vegetables, breads and dairy products necessary for proper nutrition. Our nursing staff provides assistance with eating and transportation to our dining rooms. Our meals are prepared and served by our qualified dietary staff in strict accordance with State and Federal requirements for sanitation, food preparation, servicing and storage. Our advanced computer technology enables us to print meal tickets, recipes and production reports per meal. We are also able to place food orders, print cost reports, track weight histories and control inventory.

On birthdays, residents can select a special meal of their choice such as rib-eye steak, shrimp, pork chops or chicken.  We provide special meals on holidays and special events. A variety of snacks are available throughout the day and evening. Visitors are welcome to dine with our residents for a minimal fee. A 24-hour advance notice is greatly appreciated to assure enough food for all.

 SOCIAL SERVICES

 Making the decision to live in a nursing home is a very difficult decision for everyone. Our Social Services Staff works primarily to make the adjustment to long term care a comfortable and pleasant experience for both the residents and their families.

 The Social Services Department assists our residents and families with a variety of issues including an introduction to other services within the facility, introductions to other residents, development of new friendships, answering questions, providing emotional support, assisting with paperwork needed to plan for the future and provides assistance in obtaining appropriate community resources.

 Above all else, the Social Services Department exemplifies Livingston Manor’s commitment to caring and providing excellent care to each and every one of our residents and their families. In addition to our dedicated staff, we also have a Licensed Clinical Social Worker available if needed or requested to provide additional counseling and consulting.

 Discharge planning is also coordinated through our Social Services Department. When a resident has been rehabilitated and is capable of returning to independent living, we assist residents and families with obtaining support services and referrals to community agencies to ensure a safe and successful return home.

 ACTIVITIES

 Livingston Manor’s Activity Department is staffed by a full-time Director, and four Activity Aides who provide a daily agenda of recreational programs based upon individual needs, interests and capabilities. The purpose of activity programming is to promote each resident’s sense of well being and self-worth.

 To meet the variety of resident needs, activity programs include large groups, small groups and individual activities. Programs include social and recreational groups, arts and crafts, community activities, music, and community outings, worship services and special events. Personal Services such as shopping, letter writing, hair styling and manicures are available to residents through the Activity Department.

Families are encouraged to take part in the daily activities, to participate in the Family Nights throughout the year and inform Activity staff of any special needs or preferences their residents may have.  Church groups, community organizations and committed individuals routinely volunteer their services tot he Activity Department.  Livingston Manor also has a very active Resident Council who give feedback to staff on nursing home issues, help plan and sponsor events and support community projects of interest to the residents.

All activity personnel are members of the Corn Belt Activity Directors’ Association and receive twelve hours of professional continuing education annually through this association.

 LAUNDRY, HOUSEKEEPING AND MAINTENANCE

Livingston Manor operates an in-house laundry service seven days a week.  Resident clothing is laundered daily. Our laundry staff takes great care to prevent damage and misplacement of our resident’s clothing. In order to keep everyone’s clothes from being misplaced, we require that all articles of clothing be permanently marked with the resident’s name in an inconspicuous place. If you prefer to take a resident’s belongings home for cleaning, please notify us at admission and we will notify our laundry staff of your wishes. For you convenience, nametags are available for purchase. Housekeeping services are available to all residents daily. Our housekeeping staff is thoroughly educated in all sanitary procedures.

Our Maintenance Department is on duty daily and on call 24 hours a day, seven days a week. They assist our residents in a variety of ways including television hookup, regulating room temperatures, was well as electrical and other repair work.  In addition, they maintain all of the equipment and do all of the upkeep in our 37,282 square foot building.

SAFETY

 Livingston Manor employs a Safety Coordinator who trains and performs emergency drills involving all new employees on how to respond to emergencies such as fire, tornado, etc. A safety committee is in effect that involves all county departments who discuss, evaluate and implement any changes needed to improve the quality of safety services for the residents and staff of Livingston County. Livingston manor has electronically secured doors and surveillance cameras at each nurses’ desks monitoring the exits, hallways and living areas.

ADDITIONAL SERVICES

Daily operation of the Manor is conducted by an Illinois Licensed Nursing Home Administrator. Our Administrator is available to answer any of your questions and concerns. Our Administrative office assists our residents and families in a variety of ways. Many of our residents prefer to keep their spending money in a personal trust account through the facility. Their money is available during business hours. This service safeguards against misplacement of money. We prefer that valuables such as jewelry, large sums of money and family heirlooms be given to family or placed in safety deposit box at a local bank. We also provide minimal space on the premises. The Administrative Office takes the responsibility of delivering personal mail on a daily basis.

Livingston Manor employs over 125 people in all departments who receive ongoing training and education.
